  Urine Leak (SUI)

Urinary incontinence means involuntary leakage of urine. It means inability to hold urine under certain conditions.

This is very common yet least understood and unreported complaints of women.Almost 20-30% of young women and upto 50% of elderly women suffer from it.They bring it to notice when it affects their daily chores and social life.

Women are more likely to develop incontinenece due to excessive stretching during pregnancy and child birth cn result in weakening of pelvic muscles which hold back urine. After menopausebthe pelvic floor muscles weakens and there is further worsening of  existing condition.Apart from this, weakness of nerves or muscles due to aging or uncontrolled diabetes can contribute to loss of urinary control.

Incontinence does not happen to all women and women suffering from it should seek treatment. There are different kinds of incontinence and treatment depends upon the type with which woman is suffering from.

Stress Incontinence-means loss of urine whenever there is pressure inside the abdomen increases such as while sneezing, coughing, lifting weight or lauging aloud. This is usually the direct result of weakness of the pelvic floor and common after child birth and menopause.

Urge Incontinence-means that when the person gets the urge to pass urine, she senses urging and leaks small amount of urine even before she can reach the toilet. It is often accompanied by frequency(having to pass urine frequently). It is usually the result of over active bladder muscles caused by urine infection or irritation or ne rve damage due to conditions like multiple sclerosis, spinal cord injury or alzheimer's disease. It is common among women in the fourth decade onwards.

Overflow Incontinence-It is urine leak when bladder is full and can't hold back for longer time due to unavoidable social condition. It can occur to woman of any age group but more common in elderly ladies.

Mixed Incontinence-is the presence of both stress and urge incontinence. Thus she may leak urine in response to cough, sneez and may also not be able to hold urine till she reaches the toilet. This is most common type of incontinence among women.

Urinary leaak may often be accompanied by weakness of pelvic floor muscles leading to prolapse or something coming out per vagina.

Correct diagnosis of type of incontinenece is necessary to ensure right treatment as it depends upon type of incontinence.Besides routine tests and ultrasound, cystoscopy(visualisation of inner lining of the bladder through telescope) and urodynamic study is done.

In general ,treatment of stress incontinence is surgery to support the pelvic floor muscles, while treatment of urge incontinence is medication to relax detrusor muscles. However treatment has to be individualised as each case is unique and a combination of medical and surgical treatment is often needed.

Pelvic floor muscle exercise(kegel's)help to strengthen the pelvic muscles and offer relief to many women with mixed and stress incontinence of mild variety.
